When to Sign This Form

You may sign the Meeting Attendance Verification Form if:

  • You personally facilitated the in-person meeting, and

  • The participant was present at the meeting.

You should only verify attendance for meetings you facilitated.

What You Are Verifying

By signing the form, you are confirming only that:

  • The individual attended the meeting on the specified date and time.

You are not verifying:

  • Participation level

  • Engagement or behavior

  • Progress, compliance, or outcomes

How to Use the Form

  1. The participant completes their portion of the form.

  2. You review the form for accuracy.

  3. You sign and date the form in person at the meeting.

  4. The participant is responsible for submitting the form to the requesting organization.

SMART Recovery does not collect or submit verification forms.

Facilitator Boundaries

  • You may set reasonable boundaries around when forms are signed (for example, before or after the meeting).

  • You are not required to complete additional documentation beyond confirming attendance.
